The Public/Community Health Nurse (PHN) is responsible forthe mental and physical well-being of the community by ensuring the public’shealth through health promotion, disease prevention, treatment, and education.They can work in a variety of institutions such as the community, hospitals,clinics, or long-term care facilities while collaborating with stateregulators, doctors, and other healthcare professionals to keep the communitysafe and healthy.
Essential Duties and Responsibilities:
Assesses health care trends in communities
Collaborates with emergency response planning and trainingwhen applicable
Works with public health officials to help communitiesaccess and receive care
Provides immunizations and health screenings to all populationsin the community
Provides patient and community education
Monitors and delivers high-quality patient care
Records and analyzes medical data in various communities
Evaluates the health of patients and develops treatmentplans in collaboration with medical providers and multidisciplinary teammembers
Participates in the evaluation and documentation of patientsand care
Assists in developing, evaluating, and implementingprofessional development education
Refers patients to other providers as needed
Manages budgets of public health facilities if applicable
Reviews the work of assigned team members for thoroughness,soundness of recommendations, and compliance with the standards, policies,procedures, and regulations of the CDC as well as the County Department ofPublic Health
Performs other duties as assigned/necessary
Minimum Requirements:
Active Nursing Licensure in the state practicing
Two (2) years of public health or community experiencepreferred
Complies with all relevant professional standards ofpractice
Participation and completion of Maxim's Competency programwhen applicable
Current CPR if applicable
TB Questionnaire, PPD or chest x-ray if applicable
Current Health certificate (per contract or stateregulation)
Must meet all federal, state and local requirements
Successful completion of new hire training as applicable tojob site
Understand patient confidentiality and HIPAA requirements
Ability to effectively elicit/provide information to andfrom appropriate individuals (including, but not limited to, supervisors,co-workers, clients) via strong communication skills; proficiency in theEnglish language is required
Computer proficiency required
Must be at least 18 years of age
