GENERAL DESCRIPTION
The Social Worker provides coordination of care in the home to patientsof all age groups and perform psychosocial assessments, analysis,counseling and referrals to meet the needs of the patient/family forHospice and Home Health services.
Requirements
ESSENTIAL DUTIES
Conduct initial assessments within the time frame allowed by regulatoryrequirements and/or within timeframe services are requested.
Develop and implements a plan of care that specifies:
A statement of the clients psychological, social, emotional andspiritual needs;
A Care Plan that address(es) each;
Interventions; and,
Participate in Intakes/Informationals/and Discharge Planning as needed.
Creates continuity of care across all healthcare providers involved inpatients care by accurate, timely, and clear documentation of IDG notes.Provides information reflecting patient/family goals and interventions.
Maintains positive inter-personal relationship with co-workers and othermembers of the health care team and the organization. Exhibited bytimely and appropriate communication with the physician and other healthcare team members regarding the clients needs or progress. Obtainsphysician orders as needed, in accordance with policies and procedures.
Creates and supports a safe environment for the mental and physicalwellbeing of patients.

Must be able to maintain good inter-personal relationship withco-workers and other members of the health care team and theorganization.
Understand and follows safety and infection control policies andpractices.
WORKING CONDITIONS
While performing the duties of this Job, the employee is regularlyrequired to sit and/or stand; use hands to finger, handle, or feel;reach with hands and arms and talk or hear. The employee mustoccasionally lift and/or move up to 10 pounds. Specific vision abilitiesrequired by this job include close vision, peripheral vision, depthperception and ability to adjust focus.
