Position Summary:

　　To assist with maximizing patient care within specific reimbursement structures. Assist with all aspects of coordinating patient care, under the supervision of Office RN.

　　Essential Functions:

　　Takes phone calls and acts as triage nurse. Works with Office RNs to promote accuracy in OASIS reporting and documentation. Provides input to DOPS on Performance Improvement activities. Maintains supply inventory when needed. Functions as backup for ICD10 coding. Functions as staff nurse when needed. Assists Office RNs with referral process to ensure accurate, thorough information is received prior to admission of the patient. Communicates need for verbal orders, and patient information needed to Office RNs. Verifies benefits and initial authorization, when required, and communicates same to patient and clinicians. Obtains continued authorizations from insurances for all home care disciplines. Assists with communicating daily schedules to staff and patients. Maintains contact with patients to determine continued insurance coverage or changes. Daily assessment of transmitted Hommed data, with appropriate followup with physician and patient; maintains Hommed inventory. Enters physical therapy admissions and discharges, and discusses with therapists accurate responses according to Medicare guidance.
