Job SummaryWe are seeking a dedicated Licensed Practical Nurse (LPN) to join our Comfort of Home Healthcare team! The ideal candidate will provide attentive, compassionate care to adult clients with developmental disabilities. All services will be provided within the client(s) home(s). This position requires 24/7 availability.

Responsibilities

Administer medications and treatments as prescribed by the physicianAssist in patient assessments and care planningMonitor and record patient vital signsCollaborate with the healthcare team to provide comprehensive patient careEducate both staff and client(s) on health maintenance and disease preventionMaintain accurate patient medical recordsProvide support for patients with memory care needsEducate incoming staff on client care requirements during the orientation processLead and instruct Level 1 Medication Aide courses for incoming staffAbility to create and implement client care processesProviding oversight and guidance to staff and clientsCare follow-upOrganization of medical information per clientOther duties as assigned by Community Registered Nurse (CRN) and AdministrationSkills

Ability to proficiently communicate via both written and verbal means of communicationProficient in Microsoft Office software (Word, Excel, etc.)Proficient in health coaching techniquesFamiliarity with Therap, HRST, or similar electronic health record systemsKnowledge of anatomy and common medical ailmentsExperience in long-term care and/or behavioral settings is a plusAbility to adapt to different healthcare environmentsEmployment Requirements

Active Licensed Practical Nurse (LPN) licenseSuccessful completion of a background check including: criminal record, driving record, and abuse/neglectAbility to pass a drug screeningMust have active driver license, proof of auto insurance, and appropriate and reliable means of transportationCompletion of new hire orientation and all other relevant trainings at onset of employmentJob Type: Full-time

Pay: $65,000.00 per year
