POSITION SUMMARY

　　The Licensed Practical Nurse (LPN) works under the guidance of the Registered Nurse (RN) as a member of a team to give direct nursing care to patients requiring intermittent visits. The LPN collaborates patient care with the case managing RN, physician, and patient and follows a patient specific treatment plan.

　　QUALIFICATIONS

　　Graduate of state-approved school with course in pharmacologyCurrent license in the State of PennsylvaniaMinimum of two [2] years experience in patient careReliable, independent means of transportation to make home visits. Drivers must have a valid driver's license and drive a vehicle with insurance limits as established by the state.DUTIES AND RESPONSIBILITIES

　　Clinical Skills

　　Provides skilled nursing care that includes activities such as teaching and treatments (including IV therapy- if IV certified, venipuncture and complex wound care) and health promotion practicesExecutes preventive, rehabilitation and restorative nursing procedures appropriate for the patient's care and safety

　　       C. Assesses signs and symptoms in conjunction with reporting reactions to treatments, including drugs, changes in patient's physical, mental or emotional condition to the physician         and the supervising Registered Nurse

　　       D. Evaluates the patient and home situation to determine the health teaching required at each skilled visit.

　　       E. Evaluates the patient condition and home situation to determine what assistance will be available from the designated representative in caring for the patient.

　　       F.  Instructs and educates the patient and the designated representative regarding the care requirements of the patient in the home. Conducts a comprehensive physical examination and nursing assessment of the patient at each skilled visit.

　　*Home Health LPN will be covering our Mon Valley Service areas, mostly Monday through Friday, 8am-5pm.  Flexible scheduling!  Our LPN's see a minimum of 7 patients a day.  You drive your salary:  the more patients you see, the more money you make.
