Greater Cincinnati Behavioral Health Services (GCBHS) is seeking a full-time LPN to join our team and help individuals experiencing severe and persistent mental illness. We are a large, community mental health center that offers patients comprehensive outpatient treatment, supplemental psychiatric care and case management services. This LPN is responsible for providing nursing services to assigned sites, programs, teams and patients. These duties include but are not limited to; vital signs, phlebotomy, point-of-care labs and testing, vaccinations and psychiatric medication administrationPrepare clients and assist physicians with patient exams and/or treatment - obtain vital signs, draw laboratory specimens.Provide education and training to staff, family members and other significant patient supports related to illness, disease and medications.Work closely with the teams to identify/evaluate factors leading to relapse, collaborate with team and other providers to develop and implement service interventions to ensure patient mental health stability.Document all information in the appropriate electronic health records (EHR).Take and respond to telephone inquiries.Provide feedback to patients as directed by the prescriber via face to face or telephone.Greet and escort clients to exam rooms to ensure proper flow of clinic.Coordinate client care with the appropriate health care providers and/or specialty services.Keep accurate and up to date clinical records according to the protocols - electronic medical records, medication logs.Assist in stocking exam rooms and provide a safe, clean environment of care.Maintain equipment, logs and required checks (i.e. refrigerator temperature logs, sample medication logs)Works with all members of the treatment team to provide holistic health care for all clientsProvide supportive services for medical and psychiatric providers in an integrated care settingPosition Requirements:Licensure by the Ohio Board of Nursing as a Licensed Practical Nurse is required.Graduate of an approved school of Practical Nursing.Experience in working with persons who have a chronic mental illness is preferred.GCBHS has been named a Top Workplace in Greater Cincinnati and Northern Kentucky since 2010!Please visit our website to learn more about GCBHS.EEO Employer F/M/Disabled/Vets
