  A Brief OverviewThis LCSW will work parttime (0.5 FTE) with the HealthySteps program at Cobbs Creek Primary Care as a HealthySteps Specialist (HSS). HealthySteps is a pediatric primary care program that supports healthy early childhood development and effective parenting. The HSS is an early child development expert who will join the pediatric primary care team to provide interventions, referrals, and follow-up for families with patients ages 0-3. The HSS offers screening and support for common and complex young children and parenting challenges, such as feeding, attachment, behavior, sleep, parental depression, and adapting to life with a baby or young child. Trained specialists also provide guidance, referrals, and care coordination such as conducting team-based well-child visits before, during, or following the primary care provider. Visits focus on monitoring development, social-emotional functioning, relational health, and providing anticipatory guidance.

  Additional responsibilities will consist of providing professional mental health treatment to patients and families, utilizing recognized best practice interventions. These core services include assessment of psychosocial functioning and capacities, ability to complete a mental status examination, ability to diagnose mental health conditions and provide treatment recommendations. Provide a variety of therapies to address mental health diagnoses that affect children and their families. Collaborate with many disciplines, programs, departments, and organizations in the accomplishment of these duties. Must be able to provide supervision to developing mental health providers in the treatment of children and their families, including training and direct supervision of Fellows. Also, provide clinical care to the Behavioral Health Integrated Program when appropriate on an as-needed basis.

  What you will doProfessional PracticeAssesses, plans, and implements professional interventions based upon knowledge of social work practice, human behavior, and the social environment, as well as child and family development and functioning. Interventions are directed to the requirements of patients and their families around illness and disease, hospitalization, and any continuing care that may be required over time.Performs psychosocial assessment with patients and/or family.Intervenes/treats patient and/or family based on the psychosocial assessment providing age and developmentally-appropriate counseling, treatment, and resources as indicated.Encourages and utilizes feedback from patients, families, and other staff to improve professional services. Evaluate the effectiveness of professional services and redefine goals or modalities as indicated. Organizes individual workload and sets priorities based on patient care needs and departmental standards. Meets workload standards for assigned ar as.Adheres to professional medical-legal reporting requirements. Seeks appropriate supervision on risk management issues.Conducts consultations and team-based well-child visits. Visits focus on monitoring development, social-emotional functioning, and relational health, and providing anticipatory guidance.Provides referrals and tracks follow-up, as appropriate, to help families make successful connections to key resources within the community.

  Professional CommunicationsResponsible for accurate documentation of professional services in the patient's electronic medical record.
