POSITION SUMMARY

The Lead Service Coordinator is a senior-level position in the Service Coordination department that supports Service Coordinators by helping people meet their personal needs and goals in obtaining the greatest degree of independence and inclusion possible in everyday life. They help individuals with developmental disabilities and their families identify services and supports to meet their needs. They advocate monitor, and evaluate services provided to the individual.Along with carrying out administrative, mentoring and quality assurance tasks.

ESSENTIAL DUTIES AND RESPONSIBILITIES

Manages a caseload of varying ages, disabilities, and case complexity to improve the quality of support and services by establishing strong, trusting relationships through regular contact and planning with the individual and their team to identify needs and creating an action plan to assist the person in meeting those needs.Develops community relationships and support systems and if appropriate, links resources by matching the unique support needs of individuals and families (identified in the plan) with resources in the community. Linking may involve researching existing resources, developing new resources, making referrals to collaborating agencies with information and follow-up support, preparing Utilization Review requests, service authorizations, and coordinating federal, state, county, and community programs to achieve necessary supports.Ensures quality enhancement by monitoring and assessing the quality, timeliness and effectiveness of services and support received through an ongoing review process. Monitoring occurs via phone or email contact as well as through in person meetings. In person meetings are completed Monthly, Quarterly, and Annually. These meetings can occur in a variety of settings including but not limited to individuals homes, Day Programs, community settings, an office setting, group homes, and/or residential settings. If there are findings from the monitoring, reporting processes will be followed including protocols for mandated reporting in the case of suspicion of abuse and neglect situations.Maintain accurate and timely documentation by recording all activities delivered within three business day of TCM activity and maintaining a minimum of 85 TCM billable hours per month.Provides the most up-to-date information to people we support by participating in knowledge enrichment such as completing all training as assigned, continuing ones education, and attending conferences and seminars. This may include planning, facilitating, and coordinating quarterly all staff meetings under the direction of the SC Leadership team.Mentors and assists with training Service Coordinators by providing information concerning case management practices, administrative rules and regulations, community resources, vendors, and programs. Completes administrative tasks by reviewing Requests for Expenditures and approving Level of Care and sending the assessment to the designated recipient.Assures high quality services by assisting with internal and external audits as requested; attending at least one (1) visit/meeting with each Service Coordinator quarterly and au iting one (1) file per Service Coordinator per quarter,along with generating and analyzing QA data and developing recommendations/solutions. Communicates with the SC Leadership team on quality trends or concerns identified within the SC Department and creates team unity by accepting and willingly carrying out special assignments or duties.Custodian positions in Benton and Polk Counties will affirm the success of team members assigned by recruiting, interviewing, hiring, and training new talent: providing constructive and timely feedback through performance evaluations or during everyday interactions to assigned employees; and handling discipline and termination of employees in accordance with company policy and will keep an accurate inventory of cleaning supplies.SKILLS AND ABILITIES

ABachelor'sdegree from an accredited college or university a Registered Nurse license.Four or more yearsof professional experience is required, in working with individuals with intellectual and/or developmental disabilities in a community-based setting.Twoof the four yearsof professional experience requiredmustbe met by being employed by the CHS Service Coordination department as a Service Coordinator. Previous TCM or DMH-DD case management experience will be counted as professional experience in St. Louis County.REQUIRED MISCELLANEOUS REQUIREMENTS

Must have reliable transportation and an acceptable driving record. Must use your own vehicle for transportation. Must be able to drive for work-related assignments to individuals homes to meet face-to-face with individuals on caseloads.Must have a valid drivers license. A class E Missouri Drivers license if residing in the state of Missouri is preferred, but not required.Must provide proof of personal, valid vehicle insurance.SHIFTS:FT (40 hrs/wk) Mon-Fri: 8am-5pm
