  About the Position:

  Provides and coordinates excellent quality care and healing to Graham Windham’s children and ensures they are living in a nurturing, safe, and permanent family as quickly as possible. Co-create with families’ service plans that are inclusive of all stakeholders’ input; build on client strengths; that are realistic, culturally competent, provide meaningful assistance, and are in accordance with City, State and Federal Regulations.

  Duties and Responsibilities include but not limited to:

  Ensure that children are safe and well cared for through monthly assessment in their kinship/adoptive homes. Documents foster parent/child interaction in Connections.

  Partner, develop and preserve positive working relationship with children and foster parents through regular contact and immediate response to early signs of disruption. Raise case related concerns to supervisor and/or director at the earliest moment the concerns arise.

  Gauge bonding and goodness-of-fit between child and adoptive parents through careful observation of parent/child interaction during home visits, feedback from other service providers and interviews. Work with families to adapt service plan throughout the life of the case.

  Ensure that each child is progressing toward permanency. Assist in the preparation of ICPC, surrenders, TPR, custody arrangements, etc., as needed.

  Regularly appear in family court proceedings for all adoptions file in Court.

  Attend & play an active role in Family Team Conferences for children with Permanency goals of Adoption and Kinship Guardianship.

  Contribute to the completion of every FASPs and Permanency Hearing Reports through monthly adoption and guardianship update in Connections for cases with permanency goal of adoption and kinship Guardianship .

  Documents contemporaneously permanency casework activities in Connections with 72 hours

  Process, track and submit all KinGaP and adoption subsidies for freed, TPR and guardianship cases to ACS.

  Prepare and submit adoption and KinGap packets to Attorneys in a timely manner.

  Request adoption home study/addendums when requesting subsidies.

  Develop post adoption/KinGap plans with families before discharged from care.

  Recruit pre-adoptive homes for freed children not adoptively placed through collaboration with WWK Recruiter and Case Planner.

  Photo list all children with NYS OCFS Adoption Album, who are freed for adoption but not adoptively placed.

  Assure that necessary authorizations are secured to utilize photos of Wendy’s Wonderful Kids children in line with the Child-Focused Recruitment Model as permitted by the custodial agency.

  Co-Facilitate with Permanency Supervisor adoption and KinGap groups to educate parents on the adoption/guardianship process.

  Provide timely notification to GW mental health, home finding, education and medical teams of all adoptions and kinGaPs finalization.

  Uphold excellent customer service with external and internal stakeholders.

  Submit all required documents to ACS once adoptions and kinGaps are finalized in court.

  O ther duties as assigned by Supervisor.

  Qualifications

  Qualifications:

  Requires BSW or BA degree in Social Work, Sociology, Psychology, or related field, Bilingual in Spanish preferred; (2) years minimum child welfare experience with at least (1) year of direct foster care experience; Must be able to travel within NYC Metropolitan area; Must have excellent verbal and written communication skills, time management and organizational skills; Computer literate, MS Office; NYS Connections; LTS, Ability to work with diverse populations.
