JOB REQUIREMENTS: An Integrated Mental Health Therapist at WestfieldsHospital & Clinic works with patients, families, care givers, and healthcare team members in partnership with the community to help patientsachieve optimal health goals. This person will provide patients withsupport, education, referrals, and resources through comprehensive,coordinated, patient and family-centered care. The Integrated Therapistprovides culturally appropriate, non-judgmental, compassionate carewhile maintaining professional accountability and engaging in continuousquality improvement. Comprehensive Clinical Assessments. Ensures thatall interventions are performed and documented according to professionalclinical standards. Completes clinical assessment including patient\'ssocial history, social resources, financial resources, environment,self-care capacity, mental/emotional status, strengths, and weaknesses.Facilitates establishment of plan to meet client\'s needs withpatient/family and available community resources. Makes appropriatereferrals. Assists family in accessing available resources. Informationand referral for community resources to patients and families.Establishes and maintains resource file by county. Establishes contactsat county and community agencies to maintain updated information onprograms. Maintains up-to-date knowledge of financial resources in thecommunity for assisting with complex adult cases. Assessespatient/family needs and facilitates connection with appropriatecommunity resources. Short-term, brief-intervention counseling, orpre-therapy for complex cases. Assists patient and family members inidentifying psychosocial issues/impacts or their situation, facilitatesconnection with appropriate community resources. If patient/familysituation dictates ongoing therapy needs (more than 1-4 visits) refer toappropriate community resource. Work Schedule: .5 FTE/20 hours weekly -Days Required Qualifications: Master\'s degree in social work from aCSWE accredited program and State of Wisconsin licensure (LCSW) orlicense eligible. Clinical Social Work experience with individuals andfamilies. Working knowledge of, and experience with, family systemstheory, individual and family counseling, and trauma-informed carepractices.
