This full-time position pays a contracted rate of $17.79 per hour.
Job Summary
This position exists to coordinate referral processing, program orientation, eligibility documentation, One-Stop/Career Source partner referrals and outreach activities designed to encourage and increase consumer participation. This individual will hold a pivotal role on the Consumer Support and Core Service Team by serving as the initial contact for many Core service applicants. The main goal of this position will be to facilitate an increase in the number of individuals applying for Vocational Rehabilitation services.
Primary Duties
Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions. These duties and responsibilities will be rated on the Annual Performance Review.
Provide outreach services and educates individuals and their significant others about Vocational Rehabilitation services available. Provide Outreach Services to community agencies, medical offices, religious organizations and support groups by telephoning and visiting in order to inform them of available services.
Work with affiliates to ensure that basic consumer information is collected and maintained in a common database and that it can be used to track and report program activity and changes in demographics and services offered.
Perform preliminary assessments on program applicants, which include assisting applicants through the application process by explaining documentation requirements and potential services and benefits, and coordinate receipt of eligibility documentation in order to verify applicant's appropriateness for rehabilitation services. This is done while offering choice of Florida Regional Office Counselor or DOE/DVR Counselor.
Coordinate referrals to and from appropriate One Stop/Career Source partners.
Facilitate private Orientations when requested. Collaborate with Rehabilitation Specialists and VR Counselors to ensure that all participants complete mandatory Orientations.
Additional Responsibilities
Coverage of other offices within the Unit as needed and requested by Supervisor.
Review and comply with CRC Code of Ethics Guidelines
Complete all travel requests and travel reimbursement in a timely and accurate manner, complete timesheets in a timely and accurate manner. Ensure that all expenses are turned in during the month that they are incurred.
Perform other responsibilities as assigned.
Qualifications: Education, Experience, and Certification(s )
Associate Degree (AA/AS) in Human Services preferred.
High school diploma or General Educational Development (GED) plus two (2) years work-related experience may substitute for an associate degree.
Valid driver’s license and/or access to reliable transportation to perform work-related travel required. Eligible drivers must have a good motor vehicle record (MVR).
Knowledge, Skills, and Abilities
Computer literacy skills are essential. Must be familiar with the community.
Excellent verbal communication required.
Well-developed organization skills needed.
