　　SUMMARY OF POSITION :

　　General Purpose of Job: Processes referral calls to ensure patients receive the behavioral health services or resources that best address their needs based on clinical information provided.

　　MINIMUM QUALIFICATION(S):

　　Associate's Degree

　　Two (2) years relevant experience

　　Must have excellent communication, organizational, and customer service skills

　　Must have the ability to work independently as well as collaboratively

　　Maintain certification/education as required (e.g. Nonviolent Crisis Prevention and Intervention, CPR, etc.)

　　Act 31 training completion required within 45 days of hire

　　PREFERRED QUALIFICATION(S):

　　Bachelor's degree preferred

　　Minimum two (2) years’ experience in a behavioral health setting preferred
