　　Your Job:

　　The Health Nurse Practitioner is responsible for the assessment of the all patients and participation in the development of the plan of care from triage assessment through discharge. The NP will function specialized area; caring and documenting the initial medical screening exam of the patient. You will collaborate with other members of the healthcare team in the care of the patients and participate in discharge planning and patient/family teaching. The NP will participate in department performance improvement and ongoing activities.

　　Your Job Requirements:

　　• Graduate of an accredited school of nursing with a Master's Degree or Physician Assistant Program with a master's preferred.

　　• Graduate of an accredited Nurse Practitioner program with acute care qualifications/certification required or Graduate of an approved accredited Physician Assistant program

　　• Recognition as an Advanced Nurse Practitioner by the Board of Nursing for the State of Texas

　　• Current Licensure by the Board of Nurse Examiners for the State of Texas to practice professional nursing or Current Licensure by the Board of Medical Examiners to practice as a Physician Assistant

　　• At least two (2) years experience in related nursing experience preferred

　　• Membership and active participation in a relevant professional organization. Must be current in CPR; NPR, Advanced Cardio Life Support, and current department specialized training.

　　Your Job Responsibilities :

　　• Performs an initial medical screening exam and assessment on patients as they complete appropriate documentation. Dictates admission history and physical.

　　• Based on protocols, standing orders, or in collaboration with a physician arranges for necessary laboratory, radiologic and/or other diagnostic tests to complete the assessment.

　　• Refers deviations from normal to the appropriate health care professional/practitioner.

　　• Evaluates outcomes of the healthcare plan and modifies as needed.

　　• Collaborates, as needed with other health care professionals in the formulation and initiation of a health care plan.

　　• Assesses learning needs of patients, families and staff. Plans, provides and coordinates patient/family education in collaboration with other professionals.

　　• Serves as a liaison between the inpatient and outpatient areas along with other health care professionals to develop comprehensive health care plans for trauma patients and their families.

　　• Facilities discharge and follow-up care. Dictates discharge summary.
