Responsibilities Job Summary TheRegistered Nurse (RN Infusion Nurse) provides and directs provisions ofnursing care to patients in their homes as prescribed by the physicianand in compliance with applicable laws, regulations, and agencypolicies. Also, The RN Registered Nurse job coordinates total plan ofcare with other health care professionals involved in care and helps toachieve and maintain continuity of patient care by planning andexchanging information with physician, agency personnel, patient,family, and community resources. Specific Job Duties/ResponsibilitiesProvides high quality clinical services within the scope of practice andwithin infection control standards, in accordance with the plan ofcare/service plan, and in coordination with other members of thepatient/client\'s care team from admit through discharge. Completesclinical nursing assessments in accordance with federal and/or stateprogram requirements and as required by payer Ensures thepatient/client\'s eligibility and medical necessity for services asdefined by payer source and agency policy. Develops and revisesindividualized plans of care and/or service plans according to federaland/or state program requirements with other community providers.Ensures plan of care frequency and duration meets the needs of thepatient and initiates plan of care revisions as needed with physicianapproval. Education and Experience Experience Desired A minimum of oneyear experience as an RN preferred License Requirements Must havecurrent RN licensure in state of practice. Current drivers license,vehicle insurance and access to a dependable vehicle or publictransportation. Current CPR certification required.
