We are seeking highly motivated and professional applicants with prior home health experience and a compassionate sense of urgency. What You'll Do: * Determining eligibility and initiating a comprehensive plan of care (POC) based on the needs of each patient. * Evaluating patients for eligibility for Home Health services. * Collaborating on Multi-disciplinary teams to assure a patient-centered plan of care based on patient's needs and goals of treatment. * Developing a comprehensive Home Health plan of care incorporating patient goals and expected outcomes of treatment. * Thorough documentation of physical, psychological and social assessments. * Performing a thorough medication reconciliation during the admission process and providing patient with medication instruction. * Performing routine Home Health visits as needed. Reporting changes in patient needs or status to Physician as appropriate. What We'll Love About You: * Strong electronic documentation and follow-up skills required * Computer literacy and proficiency with Microsoft Office suite and EMR system * Full-time On-Call rotation with on-call pay * Willing to travel to see clients in your area * Current Texas RN license. * Driving required. Current Texas Driver's License, vehicle, and proof of insurance.
