Consults with the physician in the development of the therapyplan of care. Provides services within the scope of practice as definedby the state laws governing the practice of physical therapy, inaccordance with the plan of care, and in coordination with other membersof the health care team. Observes and reports the patient\'s response totreatment and any changes in his/her condition to the nurse supervisorand/or the physician. Treats patients to relieve pain, develop andrestore function, and / or maintain maximum functioning using acceptablestandards of practice and evidence based techniques. Instructs andinforms participating members of the health care team, the patient, andthe family/caregivers regarding the plan of care, functionallimitations, and progress toward goals. Visits patients within theentire service area of their respective agency and other nearby LHCagencies as needed. According to Plan of Treatment, completes a progressnote for each visit, and submits progress notes per LHC policy.Completes all patient evaluations and develops the PT plan of carewithin state specific guidelines. Reports outcomes of evaluation, goals,and anticipated projected frequency of care to nurse supervisor within24 hours of the evaluation. Participates in staff conferences andcommittees as requested by the agency. May supervise home health aidesper LHC policy if allowed per state regulation. Provides supervision ofthe PTA as per LHC policy and state regulation. Participate in a weekendrotation to provide services for their respective agency and othernearby LHC agencies as needed. Prepares a written discharge summaryand/or a written summary report on patient\'s condition at least every60 days. Provides in-service education as requested by agencyleadership.
