　　Scheduled hours: 12 Hour Shifts, Nights

　　Some days and weekends required.

　　Job Description:

　　Provides basic patient care under the direction of a licensed nurse. Serves as patient’s ‘Personal Care Champion’ by assisting them with activities of daily living. Provides care & support in the home setting or hospice house with special attention to patient personal care needs in end of life care. Perform visits and/or shift work.

　　Qualifications:

　　Prefer Iowa Certified Nurses Aide

　　Maintains valid licensure/certification/registration if required

　　Current Drivers license and proof of insurance for staff performing visits.

　　Must be able to work independently, have good time management skills, and timely documentation.
