　　Baylor RN must work 12 hour shift every Saturday & Sunday with a Friday or Monday included.

　　Mercy Home Health, a member of Trinity Health at Home, has a need for a Full time Home Care Baylor RN for the greater Bucks County and some areas in nearby Montgomery County.

　　What you will do:

　　The Baylor RN is responsible to provide skilled care to the patients of Mercy Home Health. Coordinates treatment to provide comprehensive care assessments and plan of care including goals, implementation of treatment, evaluation and outcomes for patients, including discharge planning. Provides patient, family, and staff education regarding care and other therapies. Demonstrates ability to use problem solving, critical thinking and priority setting skills. Ability to perform diversified duties with time limitations with a high degree of accuracy. Computer skills preferred. Previous experience in Home Care Setting preferred.

　　Minimum qualifications:

　　Graduate of an approved nursing education program

　　Licensure as a Registered Nurse in the state of PA

　　One (1) year experience as a professional care nurse

　　Minimum of 3 years relevant experience preferred.

　　Must have current Driver’s license and reliable transportation

　　Ability to work a weekend rotation once month & 2 holidays annually
