Maxim Healthcare Staffing is seeking a Nurse Case Manager who is responsible for coordinating the continuum of care activities for assigned hospice patients and ensuring optimum utilization of resources, service delivery and compliance with medical regime.

　　Qualifications:

　　Current RN license

　　At least one year of Case Management experience preferred

　　Current BLS cardpercontract requirements

　　Current TB or Chest x-ray(if applicable)

　　Ability to effectively elicit/provideinformation to and from appropriate individuals(including, butnot limitedto,supervisors, co-workers, clients)via strong communication skills;proficiency intheEnglish language may be required

　　Computerproficiency required

　　Must be at least 18 years of age
