Veterans Inc., the largest provider of services to veterans and their families throughout New England, is looking for a professional, highly motivated, goal-oriented, skilled, and hardworking individual to fill the Healthcare Navigator position and become a valued member of Veterans Inc. team!
This specialized position is an integral team member of the Supportive Services for Veterans Families (SSVF) Team. This team rapidly rehouses at-risk or homeless veteran households while providing case management to identify and overcome barriers that could result in a housing crisis. This position is based out of our Concord, NH location and serves Veterans throughout the region.
WHAT YOU'LL BE DOING DAY TO DAY:
Receive internal referrals from the New England SSVF Team, referrals have identified healthcare barriers or needs.
Reoccurring tasks include simplifying enrollment into healthcare (Private, State, or VA), locating a PCP, assistance with scheduling appointments, educating them on how to use their healthcare coverage properly.
Networking and building connections with health care teams and homeless communities to streamline treatment to our veterans.
Assisting veterans with overcoming the common barrier of lack of transportation and/or childcare to increase follow through with scheduled appointments.
Provides education on wellness-related topics, such as smoking cessation, diabetes management, and or exercise.
Assist with finding care for major health problems, such as PTSD, Major Depressive Disorder, Substance Use Disorder, and Cardiovascular disease, and other chronic illnesses.
Following up with veterans to ensure that have continued care and assess if additional assistance is needed.
WHAT YOU MUST HAVE:
Bachelor's degree in Human Services, Social Work or other related field required.
Minimum of 2 years of Case Management, Social Work, or transferable experience.
Computer proficiency in Microsoft Office Suite and Zoom
Must be able to pass a CORI and background investigation.
Compliance with Veterans Inc. COVID-19 Vaccination Policy. Religious and Medical Exemptions are considered.
