The LCSW will be part of the care management team and responsible forassisting in the management of the continuum of care and work within anintegrated treatment setting. Will provide direct care to clinicpatients such as diagnostic assessment, individual therapy, familytherapy, group therapy, and crisis intervention to patients, as referredby physicians, and work with the treatment team to promote patienthealth outcomes.
The LCSW team member will partner with the multi-disciplinary teamutilizing best practices and relevant data. Primary elements of thesocial work role include: assessment of the psychological, financial,and discharge planning needs of the patients and families, providinginformation, support, counseling, and referrals to other levels of careand appropriate resources for a variety of age-specific populations,including neonatal, pediatric, adolescents, adults, and geriatric.
Requirements: Excellent verbal and written skills to facilitatecommunication with a variety of medical and technical personnel.Participates in continuing education, i.e., seminars, workshops andeducational materials. Computer skills for data collection, comparativeanalysis, graphic software, word processing and spread sheet usage.
Education Requirements: Must be licensed as a Social Worker with aMasters Degree in Social work. Must have taken and passed the clinicalexam for social work.
Experience: All applicants must have completed 4,000 hours ofpost-graduate, supervised social work experience.
