Field Nurse RN (New Grad Experienced)

We are currently accepting applications for compassionate, motivated Full Time, and Fee for Service, EXPERIENED NEW GRADUATE REGISTERED NURSES for our CHAA division. We strive to provide QUALITY services to our patients.

Reporting to the Coordinator Of Care (COC), the Field RN assesses, plans and coordinates home care services for assigned patient caseload and provides skilled nursing care in accordance with professional standards, regulatory guidelines, meeting agency standards, and obtaining appropriate certification.

Duties and Responsibilities

Perform patient assessments to implement the best plan of care

Coordinate resources services for assigned caseload; working with the patient, family physician

Maintain up-to-date/accurate documentation for assigned cases

Maintains a cooperative relationship among health care teams

Job Qualifications Required

Active and unrestricted NYS RN license

Excellent written and verbal communication skills

Ability to meet tight deadlines.

Valid Driver’s License and access to a vehicle (No need to drive if you can work inManhattan)

Education, Knowledge, Skills and Abilities Preferred

Proficient in use of Microsoft Office Suite tools

Bi-lingual is a plus!
