Family Practice & Counseling Network has a rich history of providing nurse-managed, integrative health services and is excited to provide mobile community health services via its new health unit: The Wellness On Wheels (WOW) mobile. The WOW will have an intended objective to:Increase access to health care in underserved communities, includingCOVID testing and vaccination on a van in the community, providing counseling for what to do if tests are positive - contact tracing, managing isolation, additional tests; andPrimary care, inclusive of preventative HIV and family planning services, behavioral health services, and oral health care on the van, and providing connection to follow-up and ongoing services in FPCN comprehensive sites.To improve health literacy, understanding of healthy behaviors, and trust of health messaging - both COVID-related, such as vaccine hesitancy, and general - in underserved communities.To increase access to benefits and services that address Social Determinants of Health, including health coverage, food, income, housing, legal, and disability services.
The Nurse Practitioner, as the leader/supervisor of the WOW, upholds the health center mission by assuring that patients receive health care that is competent, caring, cost-effective, and meets with a high degree of satisfaction; and supports the fiscal stability of the organization. Responsibilities/Duties
Provides a wide spectrum of primary care to community members/patients/students in approved mobile health care unit setting. Care includes, but is not limited to, medical assessment, diagnosis, treatment, referral and follow-up.Proficient in the management of acute and chronic conditions, both simple and complex.Practice is based on accepted evidence-based criteria and established clinical practice guidelines, utilizing an individualized, dynamic plan of care that can be applied across the continuum.Updates patient health records for accuracy at each patient encounter using appropriate designated forms and documentation requirements.Orders, interprets, and evaluates diagnostic tests to identify and assess patients' clinical problems and health care needs.Documents/dictates key components of patient's progress via daily progress note, transfer, and discharge summary, and or clinic note where applicable. Documentation is timely, meets acute care compliance standards and captures patient acuity.Consultation with collaborating physician and other members of the healthcare team when indicated.Collaborates with interdisciplinary health care team in making diagnoses of acute and complex chronic conditions and referral of services when applicable, including the patient's primary care provider (PCP) and/or specialist.Identifies expected outcomes from diagnosis and formulates and documents a plan of care to address health care needs.Implements and modifies plan of care where necessaryPrescribes diagnostic strategies and therapeutic interventions when identified.Maintains ongoing communication and collaboration with interdisciplinary care team.Facilitates and communicates with patient, family, staff, and PCP to promote continuity of care across the continuum of care.May perform advanced procedures consistent with privileges and competency validation.Responsible for the delivery of high-quality care and practice outcomes in nurse-managed school-based health center/s serving low-income student populations.Provide preventive physical and behavioral health screenings and care coordination as appropriate.Other duties as assigned Required Experience
Five years prior experience as an RN & / or Nurse Practitioner.Knowledge and experience must complement needs of the position.Proficiency in basic computer skills. Required Education

Family Practice and Counseling Network, a program of Resources for Human Development, is a network of community health centers providing comprehensive primary care, dental and behavioral health services. The Network consists of four health centers in Philadelphia - Abbottsford-Falls Family Practice & Counseling in the Northwest, the Stephen and Sandra Sheller 11th Street Family Health Services of Drexel University in the North, the Health Annex in the Southwest, and Annex West Health Center in West Philly housed in Action Wellness building (formerly Action AIDS), as well as the SBHC Collaborative, a partnership between Education Plus Health (EPH) and FPCN at CB School in Roxborough, as well as Vaux & B21, as well as other sites. Today the Network serves more than 20,000 patients annually and is an NCQA certified Patient-Centered Medical Home, and recognized by HRSA as a National Quality Leader for Behavioral Health.The Network's mission is to provide person-centered, integrated and comprehensive health services to individuals and families across their lifespan. Network services are targeted to the surrounding communities and the advisory committee that guides services primarily consists of people from the communities served.Vaccine Mandate:FPCN, a program in the City of Philadelphia, are required to follow the COVID vaccine mandate for healthcare workers perPhiladelphia Department of Public Health. FPCN requires Healthcare Workers to be fully vaccinated. Fully vaccinated means it has been two weeks since the final dose of vaccine. Thus, we require vaccination prior to start for all new hires. This currently does not include boosters.
