Maintain integrity of Electronic Medical Records (EMR) in all mediumsaccording to department policy and procedure Description Opens and sortsincoming mail. Collects hospital discharge records and uses hospitaldischarge list to reconcile receipt of paper charts. Prepares the papermedical record for scanning. Scans and indexes paper documents into theEMR. Electronically analyzes, reanalyzes, and assigns signaturedeficiencies to the appropriate practitioner in the Chart CompletionModule. Performs quality reviews of scanned and interfaced images in theEMR and verifies the quality of image, accuracy of patient name,appropriate bar code, account number and document type. Resolves issueswith scanned and interfaced documents. Maintains Turn-Around-Time (TAT)for prepping, scanning, analysis, and quality control of documents.Sorts and scans loose sheets into EMR accurately on a daily basis.Retrieves, signs out, and delivers hybrid medical records as requestedfor patient care, correspondence, studies, or audits. Performs filemaintenance and prepares records for offsite storage. Responds torequests for patient information in a timely manner. Processes and filesAdvanced Healthcare Directives in accordance with Federal and/or StateStatutes and Hospital/Departmental procedures. Performs other jobrelated duties and assignments as requested. HighSchool Diploma or GED. 12 months experience utilizing an ElectronicMedical Records (EMR) system OR successful passage of the Medical RecordDepartment Qualifying test OR 12 months continuous experience within thepast 3 years as a Health Record Clerk, Health Record Technician orHealth Record Analyst. Successful passage of basic Medical Terminologycourse OR successful passage of the CCH Medical Terminology exam. Basicknowledge of Microsoft Word, Excel, and Outlook. Excellent customerservice skills with the ability to communicate efficiently. Exceptionalorganizational skills with attention to detail. Ability to problemsolve, organize and prioritize workload to meet turn-around-timebenchmarks. Schedule Details: 40 hours per week, 8a-4p, rot. days, noweekends and occ. holidays
