Position Overview: Responsible to revaluate all potential admissions to the Manor to determine if they are appropriate for skilled nursing home placement for long term or short-term rehabilitation and oversight of social work department. Essential Duties & Responsibilities: *Work closely with hospital representatives and/or other community representatives in regards to potential new admissions. *Review all records or meeting the potential admissions at the hospital if applicable. *Review all medical and financial aspects for admissions in collaboration with the Administrator and Director of Nursing/designee for potential challenging admissions. *Meeting with all other departments in order to maintain good working relationships and open communication. *Meet with new admissions/families to assist in completing the admission agreements. *Tour potential residents around facilities. *Collaborate with team to plan for bed availability and potential admissions. *Must be able to communicate with staff, residents, and families. *Ensure SW department are meeting resident and family's needs and meeting DOH regulations and guidance. Basic Duties & Responsibilities: *Adhere to all established safety regulations and guidelines. *Maintain confidentiality in regards to resident's privacy and care. *Endorses and complies with the vision, objectives, procedures, and policies of Bethany Village. *All other duties as assigned
