　　JOB DESCRIPTION:

　　Working in the home/community environment you are responsible for the delivery of quality one on one services to consumers while implementing their goals as outlined in the Person-Centered Plan. Your care may also involve activities of daily living and communication with other members of the consumers’ multi-disciplinary team.

　　You are required to complete all documentation in accordance with state and program regulations, Aveanna Healthcare policies and procedures, and any applicable accreditation standards.

　　ESSENTIAL JOB FUNCTIONS:

　　Performed under the supervision of a registered nurse, licensed practical nurse and/or appropriate supervisor, who will be responsible for the client care you

　　Helps the client to maintain good personal hygiene including bathing, shaving, grooming, and dressing.

　　Assists in maintaining a healthy, safe environment

　　Assists the client with transfers and ambulation with or without an assistive device

　　Provides medication reminders

　　Encourages the consumer to become as independent as possible according to the nursing care plan

　　Attempts to promote client’s mental alertness through involvement in activities of interest

　　Gives simple emotional and psychological support to the client and other members of the household

　　REQUIREMENTS:

　　High school graduate, GED or equivalent of education and training

　　Clearances or Certification as required by state law and regulation

　　Demonstrated proficiency in clinical assessments, documentation and compliance with aide care and policies and procedures

　　Completion of all applicable health screening(s)

　　A minimum of two satisfactory references and background screenings

　　Valid Driver’s License and Acceptable MVR

　　PHYSICAL REQUIREMENTS:

　　Must be able to speak, write, read and understand English

　　Must have reliable transportation

　　Occasional lifting, carrying, pushing, or pulling, up to 25 pounds

　　Must be able to sit and climb stairs

　　Must be able to sufficiently reposition patients

　　Must be able to appropriately respond physically and mentally to emergency situations in the onsite or during transport, such as perform one-man CPR for a prolonged period.
