You belong here Summary of Position: Responsible forproviding the necessary support, case management and personal assistanceneeded for individuals to be independent in their home and in theircommunity. This position will support the Daily Living Skills Manager.Clients may have multiple needs including but not limited to: cognitive,mental health, physical, medical, learning, and behavioraldisabilities. 1. Education and Experience: High School diploma orequivalent. One (1) year of experience working with persons withdisabilities or in a nursing or home health care position. This positionrequires pre-employment and annual background checks. 2. Knowledge,Skills and Abilities/Essential Functions: **Following is a listing ofrepresentative examples of duties for this position; it is notconsidered to be a complete listing of all duties and responsibilitiesfor the position. Possess awareness of special needs and the ability toprovide assistance according to individual needs (i.e., medicationschedule, bathroom routines, and schedules, care procedures). Makesthese known to and trains other support staff. Work cooperatively withall people. Maintain a professional image while working in homes and outin the community. (see Code of Ethics) Possess awareness of realisticwork expectations regarding quality of work, and appropriate workbehaviors. Communicate with supervisor in a timely manner. Provide newand ongoing training for staff. Responsible for maintaining case notefiles. Ability to perform assessments and intakes for individuals new tothe program. Provide direct support to individuals in their home and inthe community. Ability to travel throughout ODC\'s service area(Wisconsin Rapids, Marshfield, Stevens Point, Wausau, etc.) Work aflexible schedule some evening hours and occasional weekend work (whenapplicable). Perform personal care in a discreet, respectful manner(i.e.; toileting, eating, mobility, behaviors). Provide physicalintervention if necessary to respond to client behaviors that could beinjurious to others. Receive training and possess the ability tocomprehend and follow through with established procedures and plans.Evaluate and report on the performance of adults with disabilities tomake recommendations concerning goals and outcomes. Observe and documentdaily progress, concerns, outcomes of each individual. Break tasks downto support individuals to meet their specific outcomes. Ability tounderstand, follow, carry out written directions, plans, and procedures.Facilitate and support community integrations for individuals wesupport. Implement training plans as established by Daily Living SkillsManager. Allow individual to become as independent as possible in allaspects. Complete first aid, CPR and CPI training and certification ifrequested to. Responsible for scheduling allotted time for eachindividual supported. Client attendance checks. Personal Care training.All other duties as assigned. 3. Physical Requirements: Ability tofacilitate, initiate and respond to verbal, written, computerized,telephone and fax communications. Requires capability to visually seeand hear to respond quickly to individuals and situations. Ability todemonstrate, train and/or model job tasks/duties/responsibilities.Ability to visually see and physically perform quality inspectionaccording to ODC\'s and/or customer\'s requirements.
