Description

A well-renowned and compassionate Homecare/Hospice provider in San Diego is seeking a dedicated Customer Service professional with a CNA background. The successful candidate will have the ability to provide superior service to both patients and their families.

Responsibilities Include:

Patient Support: Address patient queries and problems, providing prompt and effective solutions.

Care Coordination: Coordinate care with other health professionals, schedule patient appointments, and make referral calls.

Documentation: Maintain accurate patient records, queries, and interactions.

Care Escalation: Escalate complex care situations to clinical staff as necessary.

Requirements

High school diploma or equivalent; Certified Nursing Assistant (CNA) certification or equivalent is a plus.

Previous customer service experience, preferably in a homecare or hospice setting.

Excellent interpersonal and communication skills.

Working knowledge of healthcare industry protocols and patient care.
