PositionSummary: The Crisis Stabilization position serves as a member of theWraparound, REACH, and/or Youth CCS team providing individual skilldevelopment and enhancement services to support youth and young adultsreach their recovery goals. The Crisis Stabilizer serves youth atvarious stages of their recovery from mental health, substance use, andco-occurring needs by embracing the youth\'s strengths to meet theiridentified needs. The Crisis Stabilizer role varies on each team andservices are provided in the community to meet the needs identified inthe Plan of Care. Essential Functions: 1.Act as a member of theWraparound, REACH, and Youth CCS teams providing services to youth andyoung adults with mental health, substance use, and co-occurring needs.2.Build off the person\'s strengths to meet their individualizedrecovery needs. All services will be person-centered and directed by theindividual being served. 3.Provide crisis stabilization services,including individual skill development and enhancement services relatedto training in communication, interpersonal skills, problem solving,decision making, self-regulation, conflict resolution, and otherspecific needs identified in the Plan of Care. 4.Services also includetraining in daily living skills related to personal care, daily tasks,transportation, school, and accessing and connecting to communityresources and services (including healthcare services) and otherspecific daily living skills identified in the Plan of Care. 5.Utilizevarious methods to serve people including but not limited to: on-callde-escalation, role modeling, monitoring, mentoring, supervision,assistance, and cuing. 6.Provide services in the community includinghomes and other community locations. 7.On-call 24 hours per day, 7 daysper week, to provide de-escalation to youth and young adults in crisisto assist with self-regulation and problem-solving through challengingsituations. 8.Participate as a team member in team meetings with theindividual to create a Plan of Care, identify and prioritize goals, andmonitor progress toward meeting identified needs. 9.Appropriatelydocument all contact with the people being served and team...
