JOB REQUIREMENTS: JOB TITLE: Crisis Stabilizer - CCM - TCM FLSA STATUS:Hourly / Overtime eligible JOB LOCATION: Social Services - AdultPrograms PRIMARY FUNCTION: Provides stabilization and support of theclient\'s wellbeing and support of the case manager, peer specialist,and clinician to meet the client\'s crisis/safety plan. PRIMARY DUTIES:Legend: E = Essential NE = Non Essential N/A = Not Applicable Providehelp and assistance with the delivery of all aspects of individualsupport to the client such as transporting client to appointments,assist in developing a comprehensive safety plan for use during times ofcrisis, meet with the client when admitted to hospital, be available tomeet with client within 24 hours of receiving the referral, assist inlinking client to resources in the community and work with the crisisteam to assist through the recovery process (family support andcommunity based), and assist in the development of a Crisis Plan, toguide the consumer through their recovery. E Provide supportive serviceswith a person centered trauma informed focus. E Communicate withapplicable stakeholders as appropriate in a timely manner after eachcrisis incident. E Prepare, complete, and submit required items by duedates to include, but not limited to evaluation of services provided,documentation of all interventions, preventions, and stabilizationcontacts. E Attend meetings and professional development activities, asrequired. E Establish and maintain good communication, collaboration,and cooperation with all clients, parents, co-workers, etc. E Follow allduties of a mandated reporter.
