DESCRIPTION OF WORK
This is an entry level professional social service position assigned to the Adult Targeted Case Management (TCM) Unit of the Lycoming-Clinton MH/ID Program and will follow the Community Based Model. Employees in this classification participate in formal and informal agency training programs which provide knowledge of the methods, procedures, rules, and regulations necessary to perform social service and Case management services.
The Case Manager's work is performed under close supervision, but as knowledge and skills are acquired, more latitude in judgment and decision making is permitted. This position will be supervised by the Adult Case Management Supervisor.
Work Schedule and Additional Information:
Full-time employment.Work hours are 8:00 am to 4:30 pm, Monday - Friday, with a 30-minute lunch.Case management services will be available to the consumer 24 hours per day, 7 days per week through an on-call rotation during the evening and weekend hours. QUALIFICATIONSMinimum Experience and Training Requirements:
Two years of experience as a County Social Services Aide 3 and two years of college level course work which includes 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences; orA bachelor's degree which includes or is supplemented by successful completion of 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences; orAny equivalent combination of experience and training which includes 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences and one year of experience as a County Social Services Aide 3 or in a similar position performing paraprofessional case management functions.Applicants will be considered to have met the educational requirements once they are within two months of graduating with a qualifying degree/credits.Other Requirements:
