Job Description
Maintain facility and resident files, handle telephone contracts and provide direct service to residents. This position may have supervised / unsupervised contact and or conversational interaction with consumers while working in the residences, day treatments and habilitation facilities. Essential Responsibilities
Maintain all facility records and files, including resident files. This includes typing, filing, and Xeroxing.
Transcribe and type all facility correspondence and forms.
Answer all telephone calls during scheduled shift. Record all telephone calls and messages in a daily telephone log.
Make telephone contact and appointments as directed by supervisor(s).
Attend staff meetings and other meetings as directed by supervisor(s). This will require the ability to travel to other sites.
Provide crisis intervention to residents as needed.
Medication Management and Training - monitor, supervise, and record residents’ self-administration of medication.
In the absence of the cook, insure with client coordinator that lunch preparation, supervision and clean up are completed.
Assist in the provision of direct care services to resident, including planning and implementation of treatment goals and training, which will develop and enhance activities of daily living skills. These include but are not limited to:
budgeting
grooming
nutritional planning
use of public transportation/travel training
use of community resources/community integration
shopping/menu planning
housekeeping
socialization skills
physical health maintenance
assertiveness/self advocacy training
Monitor residents’ progress. Tasks essential to those duties include, but are not limited to: seeing, hearing and observing residents, performing manual tasks, reading and writing (documentation, lifting, climbing and descending stairs, traveling, boarding agency vehicles) and utilize public transportation (bus, subway) use of telephone to make and receive calls.
Ability to count/add/subtract and manage money.
Must be able to read typed and written correspondence, clinical notes, communication log, staff meeting minutes, doctors orders, fire drill logs, able to write, able to use telephone.
Communicate information with all staff and supervisor(s) verbally and in writing for the purpose of providing feedback.
Able to lift/carry packages and boxes of supplies; as well as clients if necessary.
If deemed necessary must be able to participate in these programs at various sites and varying times. The ability to complete the following training offered and perform techniques taught in each area a required function of the position; AMAP, CPR, and First Aid.
Other Responsibilities
Participate in activities of other staff members in their absence or during periods of staff shortage (i.e. cook, clerical, direct care counselor).
Represent the C.R. at meetings, trainings not otherwise specified.
Ability to work flexible schedule as work schedules and location are subject to change.
Drive facility vehicle when the need arises.
Maintain confidentiality at all times.

Job Qualifications
Qualifications
High school diploma or equivalency diploma preferred. Secretarial experience, working with MH population preferred. Experience in direct care preferred. New York State driver’s license required
