About this position Help individuals to identify and overcome barriers preventing successful compliance with their individual care plans by monitoring, evaluating, and educating customers in their home environment. The Community Paramedic will serve as customer advocates and resource coordinators in order to facilitate identification of customer needs and access to available health and community services. Bring appropriate care and supportive services directly to the customer/client out in the community empowering them to reach their health care goals and optimal wellness. Here are ou requirements Education * High School degree or equivalent (GED) required * Successful completion of Community Paramedic program * AAS Paramedic degree preferred Experience, Knowledge, Skills, Abilities * 2 years ALS experience as a Paramedic * Be comfortable and motivated to work as a care team member and autonomously with a high level of accountability. * Possess superior interpersonal communication skills. * Demonstrated ability to consistently complete through, concise, detailed documentation. * Demonstrated ability to organize, problem solve, and make decisions during emergency situations and work independently. * Possess or quickly acquire knowledge of the street and highway system within the service area.
