The Community Health Worker will primarily be working out in the community with specific target populations to improve patient care and outcomes to refugee community members and native members of Buffalo.
QUALIFICATIONS:Knowledge of the community, community organizations, and community leaders. Knowledge of the healthcare system and available social and support services. Ability to interpret, apply, and adapt guidelines and procedures. Must complete Community Health Worker training program. Experience working with low-income, diverse, multilingual populations highly desired. Ability to establish working relationships with patients, healthcare providers, co-workers, and the community, to include emotional support. Ability to apply good reasoning and judgment and react calmly in emergency situations. Working knowledge of computers. Must have reliable transportation and be able to provide transportation for clients and submit to HR permission to conduct a Motor Vehicle Report through the Department of Motor Vehicles.
COMMUNITY REACH:Conduct health and social needs assessments and follow-up to provide culturally appropriate health education, prevention strategies, and referrals, as appropriate and as identified by the clinical team and community outreach initiatives. Provide advocacy, support and follow-up to identify needs, provide basic health information, and make appropriate referrals through monthly home visits. Implement community program plans, outreach strategies, educational trainings, meetings and seminars to identify general community needs as well as services necessary for special populations.
COORDINATION:Assist in the collection of data and resources for development and evaluation of grants and maintains accurate records.
TEAM:Participate in staff meetings and trainings to improve knowledge of medical conditions and topics.
