  Position Summary

  This will be a field-based position working with the community in Detroit, MI and surrounding cities.

  Community Health Workers (CHW) serve the community as a bridge between the member (community) and the health care system (providers) through outreach and education. By providing these services, CHW's help Aetna members attain and maintain better health outcomes, improve relationships with healthcare providers and help both parties become more acculturated with each other. This position will work under the direction of a designated professional.Required Qualifications

  Must be located in Detroit, MI or surrounding counties.

  Must be willing to travel within the 90 mile radius from Detroit, MI at least 70% of the time.

  Must possess reliable transportation.

  1-2 years’ experience providing clinical services.

  3-5 years relevant experience (such as community outreach or home visits to members/patients).

  Experience in serving the Medicaid Populations in urban or rural environments with familiarity of local formal and informal resource networks.Preferred Qualifications

  Experience working in community/outpatient setting.

  Community Health Worker Certification or equivalent training and experience.

  Preferred skills: Mental Health First Aid, Trauma Informed Care, HIPAA, C.L.A.S. Standards, Motivational Interviewing, and specialized training in medical terminology and practices for designated health conditions.

  Meet all requirements set forth by local state i.e. first aid, CPR, TB screen, etc.

  Experience using a computer including the use of Excel and Electronic Health Records.Education

  High School Diploma or G.E.D.

  Pay Range

  The typical pay range for this role is:

  $18.50 - $35.29

  This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
