　　Community Health Worker – POP HEALTH HEALTH EQUITY WA SMMC WALLA WALLA 4 at St Mary Medical Center-Walla Walla

　　The Promotore De Salud Community Health Worker works to remove barriers to care, build connections and assists in the navigation of the health care system. This position works with in community focused team and reports to the Manager of Population Health SEWA

　　Shift: Average 40 hours per week, Day shift, Full-time position.

　　Required Qualifications:

　　Driving may be necessary as part of this role. Caregivers are required to comply with all state laws and requirements for driving. Caregivers will be expected to provide proof of driver license and auto insurance upon request.

　　Preferred Qualifications:

　　Bilingual

　　Experience in Community Outreach

　　Associate's Degree in Health or Social work related field.

　　Bachelor's Degree in Health or Social Work Related Field
