About the Role

The Community Health Associate II (CHA II) conducts direct outreach for Cedars-Sinai Medical Center designated communities in an effort to provide education and build professional bonds, relationships, and sustainable advisory boards and partnerships. As a community health associate, you will be accountable for creating and integrating identified health programs while partnering with health organizations and clinical staff. Your knowledge of current community programs and resources will be instrumental in developing well-targeted and effective programs. This position, under the supervision of department management is responsible for bringing together important information, resources and evidence-based awareness and screening programs to the specific identified communities. In addition, you will be responsible for data collection when necessary and will represent the department in community settings. The CHA II will also collaborate with colleagues and experts to conduct trainings in the community. This position will provide feedback to the director(s), principal investigators, clinicians and researchers, and/or administrators on community priorities, needs and barriers on a consistent basis to help catalyze the development of research projects and needed community programs.

Primary Duties and Responsibilities:
Develops and manages relationships with key community organizations and stakeholders.
Coordinates the community grants program and ensuring deadlines are met for assigned community partners.
Conducts presentations and writing publications for the community.
Conducts evaluations and assessments of community outreach events.
Responsible for data analysis and report preparation as needed.
Social media development for targeted departmental activities.
Department-Specific Duties:
Coordinates outreach activities -
Plans, coordinates, and performs various outreach activities including, but not limited to, site visits to various communities, community-based organizations, conferences, health fairs, etc. to build awareness of the Goldrich Center, memory care and research opportunities.
Serves as a representative of the Memory Disorders Program to patients, families and community organizations by attending community meetings, representing the program at community events, and making presentations on behalf of program.
In collaboration with staff, participate in development and implementation of patient/family health education.
Acts as the point person for all outreach events, such as set up, securing space, transporting materials, ordering food, etc.
Flexibility to work evenings and weekends for events as needed.
Independently oversee and manage outreach activities.
Proactively oversee the center’s outreach activities and needs.
Ability to look for local events to attend/participate independently.
Develop collaborative relationships with community-based organizations -
In conjunction with leadership and program staff, works with community agencies for developing and collaborating in new or existing partnerships.
Supports regular communication to the community and with partners.
The program is especially focused on engaging the LGBTQ+ and AAPI communities.
Outreach Data -
Prepares programmatic outreach data such as outreach activities completed, total patients/families reached, etc.
Collaborate with community and/or education leaders in the distribution of program/organization materials.
Research Recruitment -
Shares and discusses current Goldrich Center research opportunities with the public and community-based organizations at various outreach events.
Shares ongoing research opportunities with community-based partners.
Ability to speak to scientific topics as it relates to research opportunities.
Assists outreach event attendees in signing up to receive more information on research opportunities.
Qualifications
Requirements:
High School Diploma/GED is required.
Bachelor's/Master's degree in Public Health, Community Health, Nutrition, Exercise/Social Science, or related field is preferred.
California driver's license is required.
Transportation resource to be able to travel to outreach events throughout the LA area is needed for this role.
3 years of experience implementing community based programs and/or working in a community health setting is required.
Proficient in Microsoft Office Suite.
Good communication skills.
Cultural competency.
Physical Demands:
Ability to carry program and marketing materials to and from community events; setting up table/chairs at events
