　　Key Responsibilities:

　　Assist Licensed Practical Nurse or Registered Nurse in gathering medical history information and

　　taking vital signs.

　　· Documents patient information for medical record including vital signs, height, and weight.

　　· Assist Physicians and nursing staff members with procedures when necessary.

　　· Transport patients safely using appropriate equipment.

　　· Assist with stocking and cleaning clinic equipment and supplies.

　　· Follow emergency procedures including CPR when necessary.

　　· Perform other duties as assigned.

　　Qualifications :

　　High school diploma or equivalent required.

　　· Successful completion of state-approved certified nursing assistant training program required.

　　· Current/valid state issued Certified Nurse Assistant certification required.

　　· Basic Life Support (BLS) Acceptable credentialing bodies and certifications include American

　　Heart Association Basic Life Support for Healthcare Providers or Advanced Care Life Support

　　(ACLS) required.
