  Summary The Oklahoma City VA Medical Center, Pharmacy Service, is currently recruiting for one (1) Clinical Pharmacist Specialist - Oncology position in Oklahoma City, OK. The incumbent is a licensed pharmacist operating with an individual scope of practice allowing for an advanced practice functionality in the care of Veterans in this health system. Because the position will have a scope of practice, the position may also be referred to as a Clinical Pharmacist Practitioner, Oncology. Key duties for the CPP, Oncology, include: providing for the pharmacotherapeutic needs of oncology patients and serve as a pharmacotherapy expert in support of partners in oncology and throughout the health care system. The CPP, Oncology is an advocate for evidence-based, patient centered, and cost-effective medication therapy for oncology patients within the health care system. Major Duties and Responsibilities: Direct and Indirect Patient Care Functions: Clinical Pharmacist Specialist, Oncology uses expertise in oncology to optimize patient care as an advanced practice pharmacist and is authorized within the guidelines of a scope of practice to function as a provider of services to patients within the area of oncology. Direct patient care activities will be through face-to-face or non-face-to-face encounters and documented as per medical center policy. Interfaces with physicians and other health care professionals in the care of patients. This includes direct assistance for other providers in rational medication selection by providing written and/or verbal consultation. Under the scope of practice and appropriate physician involvement, designs and monitors therapeutic plans. This includes the choice of medication, dosage, laboratory tests, and therapeutic endpoints. The work is accomplished by gathering all necessary patient-specific information and through the use of accepted clinical practice principles. Each plan should be consistent with policies and guidelines established at the national, VISN, and local levels. Recognizes adverse drug reactions and makes appropriate changes in therapy. Evaluates the drug literature to compare- and-contrast therapeutic regimens. Is responsible for planning, developing, and maintaining effective clinical pharmacy programs that advocate for rational, cost-effective drug therapy to improve patient outcomes for oncology patients. This occurs in coordination and partnership with appropriate services/initiatives in the OKC VA Health Care System. Establishes and expands the patient data base through interpretation of medication history, physical examination, and ongoing patient specific monitoring of drug therapy and documents this information in the formal patient medical record. Uses dashboards operated through the national, VISN, and local levels to identify patients appropriate for pharmacotherapy service and identify changes in process needed to improve quality of care. Serves as a liaison for pharmacy service. Education and Training Functions: Assesses and provides for the educational needs of the patient and family to help ensure that pharmaceutical outcome goals are met. Provides specialized education and training support to other professional services in the broad area of advanced pharmacotherapeutics. Serves as a clinical resource in the broad area of medication use in patients. Supervises pharmacy students on assigned rotations, as scheduled. Supervises pharmacy residents or fellows on clinical rotations and/or other learning experiences. Designs and supervises implementation of interdisciplinary programs in education, training, and clinical practice with pharmacy, medical, dental, nursing, and research services. Performs in-service training as scheduled. Dispensing: Evaluates patient medication orders and dispenses appropriate medications and dosage forms, properly labeled after reviewing for completeness, eligibility and appropriateness of drug, dose, route, strength, quantity to be dispensed, reviews for drug incompatibilities, inappropriate combination, potential adverse drug reactions, and dosage ranges. Provides strict security and accountability of all controlled substances. Conducts accurate perpetual inventories utilizing prescribed record keeping and audit procedures. Provides quality control for labeling, prepackaging, compounding and dispensing activities of controlled substances by pharmacy service personnel. Qualifications BASIC REQUIREMENTS: Citizenship. Be a citizen of the United States. (Non-citizens may be appointed when it is not possible to recruit qualified citizens in accordance with chapter 3, section A, paragraph 3g, this part.) English Language Proficiency. Clinical Pharmacist Specialists must be proficient in spoken and written English in accordance with VA Handbook 5005, Part II, Chapter 3, Section A, paragraph 3j. Education. Candidates must meet one of the following: Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. OR, Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL iBT). Licensure. A full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. GRADE REQUIREMENTS: Clinical Pharmacist Specialist GS-13 Experience. In addition to the GS-12 requirements, The Clinical Pharmacist Specialist must have 1 year of experience equivalent to the next lower grade level. AND, The Clinical Pharmacy Specialist (CPS) functions at the highest level of clinical practice, works independently under their scope of practice as defined by the individual medical center to directly care for patients. A CPS plays a defined role in budgetary execution and serves as a mid-level provider who functions to initiate, modify or discontinue medication therapy and as a consultant for intensive medication therapy management services. This includes, but is not limited to, the following: designing, implementing, assessing, monitoring and documenting therapeutic plans utilizing the most effective, least toxic and most economical medication treatments; helping achieve positive patient centric outcomes through direct and indirect interactions with patients, providers, and interdisciplinary teams in assigned areas; performing physical assessments; and ordering laboratory and other tests to help determine efficacy and toxicity of medication therapy. AND, In addition to the experience above, the candidate must demonstrate all of the following KSAs: a. Ability to communicate orally and in writing to persuade and influence clinical and management decisions. In addition, the candidate must demonstrate the professional KSAs listed below. b. Expert understanding of regulatory and quality standards for their program area. c. Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise. d. Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy. e. Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters.
