Job Overview:A TriHealth Clinical Pharmacist is responsible for providing optimal pharmaceutical care to all patients we serve through distributive and clinical functions. The Clinical Pharmacist collaborates with other health care providers to ensure appropriateness of all elements of pharmaceutical care and independently operates within policies and procedures to manage the department's clinical and consultative services. The Clinical Pharmacist supervises the work of supportive team members to ensure accuracy and efficiency of distributive services. Reports medication errors and assists in assessing the risks and stratigies to manage the risk. Assists in protocol development, implementation, and optimization.Undertakes projects that will improve the quality of patient care and promote cost effective use of medications.Provides drug distribution skills include verification, order entry, clarification as needed, accuracy of checking, technician supervision, and timely delivery. Utilizes clinical knowledge and experience to resolve health problems through rational use of medications. Manages the drug inventory by adjusting par levels, and avoids stock-outs and decreasing waste. Communicates with the Pharmacy Buyers to ensure supply matches demand. Ensures complete, accurate, and timely documentation.Takes the lead in identifying opportunities and solutions in pharmacy process improvements. Applies known solutions to a variety of predictable problems. Assumes personal responsibility for professional development, particularly in the area of ongoing clinical education, and develops clinical pharmacy programs according to policies and regulations.
