　　Responsibilities

　　Direct the overall operation of the clinical services within the Home Care Home Based (HCHB) system

　　Provide oversight coordinating referrals to the agency with internal and external referral sources

　　Direct and follow established policies and procedures as related to the scope of practice

　　Maintain continuity of patient care in a home care setting

　　Proactive, collaborative team member in the home care environment

　　Current, active, and unrestricted Registered Nurse (RN) licensure in the state of practice

　　Two (2) years recent practical experience as an RN

　　One (1) year experience in Home Health required.
