Job Overview
The Clinical Care Coordinator is a Registered Nurse (RN) who manages aspecific panel of patients identified as having rising risk or high risk. TheClinical Care Coordinator is involved in the coordination of care for adesignated panel of patients across the continuum of care, as well asidentify and monitor patients with rising risk. The Care Coordinator utilizesthe nursing process and serves as the leader and driver of care coordinationefforts for their panel of patients, in partnership with themulti-disciplinary team. The coordinator ensures that the care provided ispatient and family centered, evidence-based, cost-effective, holistic innature, and leads to better outcomes. The coordinator is responsible forpartnering with the patient and family for seamless transitions in carebetween the hospital, home, school and community.

Department Name: Oncology, Solid TumorJob Status: Full-timeShift: 4 days a week, off Tuesdays.This coordinator supports patients who are diagnosed with a solid tumors. TheCCBD coordinator supports all elements of care coordination including but notlimited to appointments, IV push chemotherapy administration, central linecares, home care and education.
Qualifications
Education: Bachelor of Science in Nursing (BSN)Experience: Four years nursing experienceCertification(s):BLS/CPR from the American Heart Association with at least 6 months leftbefore expiration is required upon hire.Achievement and maintenance of certification in one of the two categories:Certified Case Manager (CCM) through the Commission for Case ManagerCertification (CCMC), Accredited Case Manager (ACM) through theAmerican Case Management Association (ACMA) or certification in CareCoordination and Transition Management (CCCTM) through the American NursesCredentialing Center (ANCC).A plan to obtain certification will need to be in place with the teammember's direct leader within 6 months of hire with the expectation that thecertification exam will be taken once all a crediting body eligibilityrequirements are met.ANCC or Magnet approved clinical specialty certification inclusive of carecoordination activities as approved by the Director of Patient Care Services.A plan to obtain certification will need to be in place with the teammember's direct leader within 6 months of hire with the expectation that thecertification exam will be taken once all accrediting body eligibilityrequirements are metSuccessful completion of required internal Care Coordination training withinsix (6) months of hire or transition into role.Licensure(s): Registered Nurse (RN) licensure in the state ofColorado is required.Professional Membership(s): Active membership in a professional nursingor healthcare organization is required.*This position may require a BLS certification from the American HeartAssociation.
