Exciting hybrid position opportunity in which the LPN will assist with both Transition Care Clinic duties and act as the coordinator for the FirstHealth Lung Bus, lung cancer screening program. Transition Care Clinic duties: While scheduled in clinic, LPN will spend the mornings assisting with patient check in/check out, answering patient phone calls, scheduling of appointments, assisting with referrals. LPN will also be trained to assist with direct patient care when needed; obtain patient history, basic vitals, ECG and assist with physical assessment. Experience with phlebotomy preferred, will train if needed. Lung Bus Coordinator duties: While scheduled in clinic, LPN is expected to spend the afternoons reviewing patient eligibility report provided, assist with uploading of appropriate reports into patient records, working directly with the central scheduling department to assist with prior authorizations and scheduling of CT scans, work with provider on patient case management and referrals. On days the Lung Bus is scheduled for community events, the Lung Bus Coordinator is expected to be onsite with the bus. Position will require the ability to travel to any of the FirstHealth market areas, which currently includes 15 counties. While onsite with the Lung Bus, the Coordinator is expected to assist with education and eligibility screening for walk-up patients, patient registration in Epic and scheduling for the CT technician. Skills Required: + Strong verbal and written communication, interpersonal and organizational skills required. + Must be able to work independently. + Ability to maintain a positive attitude and motivation with change and improvement opportunities. + Some evening and weekend work may be required. Working Conditions: + All locations of FHC properties and facilities. + Able to drive extensively through the FHC market with the ability to get in and out of a vehicle many times daily. + Flexibility in scheduling to include occasional early morning, late evening and weekend events. + Able to organize and complete assignments using mobile communication devices while in the field. Current LPN NC license required. BLS certification required. Minimum of 2 years of experience as LPN is preferred. Valid NC driver's license with good driving record required.
