Position Summary The role of the Client Outreach & Program Planner/Income Support Coordinator is vital in two important aspects: engaging with the community and planning/strategizing income support programs. Key responsibilities include: Participate in development of methods for analyzing, evaluating, and monitoring Canvas Health Certified Community Behavioral Health Clinic (CCBHC) programs and services that serve clients who are on Medicaid. With input of leadership and program managers, plan, create, implement, and execute an outreach plan related to our status as a Certified Community Behavioral Clinic (CCBHC) and how services can be utilized by active-duty military and their families or veterans, and clients who use Medicaid to access care. Assist in providing comprehensive SSI/SSDI application assistance for eligible individuals who are experiencing or at risk of homelessness. Keep in contact with the applicant, Social Security field offices, and appropriate state agencies. Represent Canvas Health in the community by establishing and nurturing relationships with various community members and educating stakeholders about the agency's services and programs. Minimum Qualifications High School Diploma/GED or higher. Two (2) years of experience working directly with individuals experiencing homelessness who have a serious mental illness and/or co-occurring substance use disorder. Evidence of ongoing training and education in related areas such as mental illness, substance abuse, and/or homelessness. Experience with providing outreach and successful engagement to a diverse population, including working with clients, developing trust, and conducting interviews in non-traditional settings and unstructured environments. Ability to work in non-traditional settings and unstructured environments. Preferred Qualifications Bachelor's degree or higher in behavioral health, public administration, stakeholder relations public relations, communications, or public health SOAR (SSI/SSDI Outreach, Access, and Recovery) training. Familiar with Social Security Administration's Listings of Impairments. Cursory knowledge and ability to write using medical and psychiatric terminology. Experience with providing outreach and successful engagement to a diverse population, includi g working with clients, developing trust, and conducting interviews in non-traditional settings and unstructured environments. Experience working with and/or delivering services to diverse populations.
