AWARE seeks to provide opportunities for employees to grow professionally through its tuition advancement program, professional development opportunities and in-house trainings.Principal Duties & Responsibilities:Provides case management services to clients.Collects data from collateral sources.Reviews relevant records and other pertinent data and resources.Participate as a member of a treatment team in the planning, formulation, implementation, and review of treatment, Personal Support or Individual Service Plan.Provides and documents services as approved and directed.Provide services to identified children and families in order to assist in gaining access to:Medical ServicesSocial ServicesEducational ServiceVocational ServicesRecreational ServicesCaseload and/or billing expectations and requirements must be met on a monthly, quarterly, and yearly basis.AWARE empowers its employees to operate under the auspices of AWARE’s Unconditional Care Principles.RequirementsRequired Skills, Abilities & Education:A thorough understanding of human services, resources, and tools.The ability to assess clients (and families) from a strengths model perspective, along with the skills, knowledge, and abilities to develop a theoretical perspective.The ability to work with a variety of people.Strong oral and written communication skills are a must.Preferred Skills & Abilities & Education:A BS/BA Degree in a Human Services Related Field is preferred; an equivalency can be used based on documented work history, education, and volunteer experience.To learn more please visit www.aware-inc.org.
