  JYF Thompson Non-Secure PlacementProvides case management services in a 12-bed Non-Secure Placement Program in the Hunts Point neighborhood of the Bronx, NY for adjudicated youth, ages 12-18 to support them as they transition safely from residential placement back into the community.Non-Secure Placement is part of a more recent initiative to provide services and supports for juvenile justice involved youth in residential placement facilities closer to their families, communities and schools.

  The Case Planner will maintain a caseload that will not exceed 12; 8 youth placed at the facility and 4 youth in the community on aftercare. The role of the NSP Case Planner is to streamline services and service plan goals, guide the discharge readiness planning beginning once youth are placed, and follow through with the youth/family in the application of the service plan in the community. Case Planner is responsible for developing and maintaining meaningful contact with youth, their families, and service providers for medical, mental health, social, educational, and other related service needs. Additionally, Case Planner will work with interdisciplinary team to lead discharge planning and ensure coordination of service delivery throughout the youth’s commitment in the NSP program. The aim in programming is to create a nurturing, supportive environment which redirects the youth to make positive life changes. This will be achieved by promoting interventions and rehabilitative approaches that are evidence based/informed and therapeutic. The goal is to reconnect youth with family and community while enhancing public safety.

  Responsibilities

  Case management responsibilities include all necessary tasks to complete case file reviews, case planning, family engagement, and group service delivery.

  Performs all NSP case management related responsibilities.

  Connects youth and families to needed services in the community.

  Attends all court hearings as a liaison and makes recommendations in court.

  Documents case contacts and other required paperwork in an accurate and timely manner in Connections.

  Ensures all ACS, OCFS, mandates and requirements are met.

  Works collaboratively with interdisciplinary team and other program/division/support staff.

  Works effectively with other professionals in the field.

  Maintains knowledge of the juvenile justice system and issues affecting youth.

  Supports, and participates in, the group interaction process.

  Conducts regularly scheduled and unscheduled home assessments.

  Has flexibility to work some weekend and evening hours based on program needs

  Develops and maintains supportive relationships with youth and families, engaging them to be a part of the service planning and change process.

  Understands the YLS (Youth Level of Service) Risk, Needs & Responsivity framework and is able to identify criminogenic need and responsivity factors.

  Responsible for developing discharge plan with interdisciplinary team while incorporating youth and family input

  Coordinate & schedule visits within the facility with families and other stakeholders

  Supervise family visits including interaction between youth and family as needed.

  Participate in all required meetings including but not limited to treatment team meetings, staff team meetings, administrative team meetings, family team conferences, recreational and family engagement activities (i.e. Community Team Conferences, Family Night and Release Ceremonies).

  Collaborate biweekly with the Placement & Permanency Specialist (PPS) about the youth’s adjustment and progress in program and while on aftercare, including any high risk/safety concerns.

  Coordinate and network with community resources to create opportunities for youth/families. Assist youth with enrolling in any required aftercare programs.

  Maintain clear and concise documentation that includes discharge planning, engagement, assessment, child well-being, safety and progress while in placement. Complete all FASPS, progress notes, plan amendments, and any other reporting/case management requirements via Connections accurately and in a timely fashion (within 7 days).

  Participate in all agency, ACS/CTH and program training and weekly individual supervision.

  Model reliable, supportive and positive relationships to help youth navigate challenges and achieve their goals.

  Model professionalism and respectful behavior in both the community and in facility.

  Advocate for youth and families with external stakeholders

  Complete and submit timely PYA checklists

  Collaborate with Credible Messenger – Aftercare Team to continue to provide case management services to youth on aftercare.

  Travel: NSP youth/families reside in NYC and surrounding areas. The Case Planner will be required to travel, navigate NYC/surrounding neighborhood roads/public transportation system and complete casework contacts in families homes, NSP facilities, court, schools, and other service providers. The Case Planner is also required to drive, transport youth with other staff members and escort family members. The Case Planner will provide their own transportation and work-related travel expenses will be reimbursed.

  35 Hrs/WK

  Daily schedule and hours will fluctuate to meet the needs of the program. The Case Planner will be responsible for maintaining their own schedule appropriate to the needs of the program and youth served, which include day, evening and occasional weekend hours.

  Qualifications

  Bachelor’s in Human Services degree.

  Knowledge of child welfare and juvenile justice systems

  Demonstrated knowledge of Family Court procedures

  Bilingual (English/Spanish) a plus

  Excellent verbal, written communication and organizational skills are essential

  Proficient in Microsoft Office suite and NYS Connections

  Current and valid driver’s license.

  Willingness and flexibility to work with youth/families in their own homes and in the community.

  Strong interpersonal/communication skills.

  Ability to intervene decisively & physically, when necessary, to protect youth and family members.

  Ability to set appropriate boundaries and limits for best practice.

  Must be organized and self-motivated; must be able to work independently and cooperatively.

  Salary 55,000/Annually
