Women's Navigator - Case Manager - Substance Use Disorder

　　Monday - Friday 8:30a to 5p

　　Case Management Services support individuals impacted by substance use to achieve and maintain optimal functioning in the least restrictive, most normative community setting. Case Management meets the personal, interpersonal, occupational/educational, and social needs of adults diagnosed with substance use and/or co-occurring disorders (substance use and mental health disorders).

　　As a Women's Navigator, you will be critical in providing support, guidance, and advocacy to women seeking help with substance use disorders. Your primary responsibility will be to assist female clients in navigating the complex landscape of substance abuse treatment and support services.

　　Key Responsibilities:

　　Conduct outreach to identify individuals needing substance use disorder and MAT services.Perform substance use screening and needs assessments.Develop treatment plans tailored to each client's unique needs.Facilitate linkages and referrals to SUD/MAT services.Provide advocacy, supportive counseling, crisis intervention, and daily living support.Offer medication supervision and monitoring.Collaborate with local correctional facilities, hospitals, Human/Social Services offices, and other community settings.Conduct community outreach and education to inform community partners and stakeholders about available substance use treatment services within H-NNCSBRequirements:Bachelor's degree in Human Services, such as Social Work, Psychology, Sociology, or Counseling. Minimum of one (1) year of substance use disorder (SUD) case management experience. Certified Substance Abuse Counselor (CSAC) certification is a plus.Valid Virginia Driver's License.

　　The selected candidate must successfully pass a criminal history fingerprint background investigation, DMV record check, Child Registry search, drug screening test and employment reference checks.
