EAC Network, a not-for-profit social service agency that empowers, assists, and cares for over 62,888 people in need through 100 programs across Long Island and NYC, seeks a Case Manager to work full-time for the Suffolk Community Services program.
The Case Manager position is a non-exempt position paying $18-21 per hour.The position is full-time with a work schedule of Monday-Friday 9AM-5PM.The office is located in Hauppauge, NY. The role reports to Program Supervisor.

Primary Purpose of Job:
The Case Manager is responsible for working directly with offenders involved with the Community Service Program. The Case Manager conducts screenings/intake interviews, refers clients to appropriate volunteer worksites, and monitors these individuals to completion.
Principal Duties & Responsibilities:
Conduct client intake interviews and collect client fees.
Match clients to appropriate worksite placements and conduct client follow-up.
Identify and link clients with supportive services.
Maintain accurate client records.
Log information in the program database according to the established timeframe.
Monitor clients while they are performing their community service assignment and report any problems, issues or concerns regarding the clients progress.
Submit client completion and other required forms for submission to the community justice referral source.
Assist in maintaining information systems and tracking of referrals.
Provide required client information to ensure prompt entries into database.
Knowledge, Skills, & Abilities Required:
High School Diploma or educational equivalent required, minimum of an Associates degree preferred.
Must be proficient in computer applications or other automated systems such as spreadsheets, Microsoft Office applications, calendar, and email and/or database software necessary to perform work assignments.
Some knowledge of services for the disabled helpful.
Excellent communication, organizational and interpersonal skills.
Must possess a valid NY State Drivers License and have access to an automobile.
