CASE MANAGER

Homes for the Homeless (HFH) is searching for an experienced Case Managers for our City Sanctuary Facility for Families w/ Children in Staten Island, NY. As the Case Manager, you will play a pivotal role in our organization, assisting families in providing them with the necessary care and resources needed. 　　Homes for the Homeless, Inc. (HFH) is a non-profit social services organization operating family shelters throughout NYC that serve as a hub of opportunity for families both residing in shelter as well as those in the neighboring community. On-site programs range from childcare, early childhood education centers, afterschool clubs and programs, and youth recreation activities, such as sports teams, to housing assistance, case management, parenting workshops, career and job vocational training, support groups and substance abuse assistance, and family engagement and recreation events. 　　ESSENTIAL DUTIES AND RESPONSIBILITIES

　　Maintain and update digital and physical records of all families as required by governing agencies.

　　Collaborate with clients to develop service plans based on an assessment of each family’s needs.

　　Assist families with connecting to community resources, support groups, and cultural services to provide social support.

　　Assisting families with referrals to outside agencies for legal services, education, parenting, medical, and mental health services.

　　Maintain and update case files bi-weekly to document progress.

　　Adhere to all city and state guidelines for maintaining documents.

　　Conducts unit inspections.

　　QUALIFICATIONS

　　Bachelor’s or Associate degree.

　　Fluency in languages other than English, particularly Spanish strongly preferred.

　　Knowledge of Outlook, Word, and Excel
