The Case Manager Re-Entry, is responsible for access, reentry, and case management services to individuals, primarily to ex-offenders, who are participating in chemical health treatment and recovery maintenance/continuing care beginning with in reach while incarcerated and transitioning to treatment and recovery services in the community. PRIMARY RESPONSIBILITIES: Demonstrate commitment to SAMSHA's definition of recovery and recovery principles: "A process of change through which individuals improve their health and wellness, live a self-directed life, and strive to reach their full potential." www.samhsa.gov Meet with clients while incarcerated. Develop and monitor reentry plans. Update reentry plans as needed. Provide on-going assessment and referrals. Provide service in clients in correctional facilities, in the community, including in participant homes. May facilitate groups with the clients of the program. Develop and maintain relationships with other providers within the community. Actively participate in the division's Trauma Informed Care Initiative and motivational interviewing. Support and implement evidence-based practices related to specific program area. Employ a holistic approach including promoting health and wellness activities with participants. Participate as a partner on the team; contribute to discussions, actions and projects. Qualifications REQUIREMENTS: Bachelor's Degree in Social Work or similar field or 3 years experience in a similar role/function. Minimum 1-2 years of experience in a similar role/function working with the prison population. Minimum one year freedom from alcohol and chemical abuse addiction. Experience with and/or commitment to working in a diverse, continuum of services in a fast-paced, growing organization. Experience with and/or a demonstrated commitment to developing and delivering culturally responsive services. PREFERRED: Ability to work independently with participants in home or community settings. Previous work experience in a behavioral health setting. Work experience in a culturally diverse environment. Experience with programs serving individuals with dual diagnoses.
